Registered Nurse (Adult Medicine)

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.

The Staff Nurse is a key team member responsible for providing an environment in which a patient can receive competent, comprehensive, culturally sensitive and age appropriate care. In order to fulfill these responsibilities the nurse must bring to her/his role, sound clinical judgment along with communication, leadership and organizational skills. S/he will be responsible for the delegation to and supervision of other licensed and non-licensed support staff. S/he will collaborate in a confidential manner with the paten, the patient's family and other members of the care team in order to optimize patient outcomes. The RN is expected to demonstrate the knowledge and skill necessary to provide care based on physical, psycho/social, educational, safety and related criteria appropriate to the age of the patients care is provided to in his/her assigned area as per approved standards and criteria.

JOB DESCRIPTION

Summary/Objective:

Manages individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, coordinating cross continuum care and assessing outcomes of interventions in accordance with system, departmental and ambulatory policies and procedures. This role provides clinically-based, risk reducing care and coordination that facilitates the delivery of cost-effective quality healthcare along the continuum. It directly interfaces with physicians, care managers, health care teams, patients and their caregivers to achieve desired patient outcomes.

Essential Functions

JOB KNOWLEDGE - The extent to which the employee demonstrates technical and professional knowledge of job requirements and takes personal responsibility to maintain and improve that knowledge:

The employee is able to fulfill all of the requirements of the job assigned in a satisfactory manner as demonstrated by the responsibilities below. Nursing assignments are made weekly, based on staffing and provider schedules. All nurses are expected to be able to perform all the tasks listed. Tasks may include but are not confined to the following --

1. Floor:

  • Hearing and vision screens
  • CLIA waived labs: UA, pregnancy tests, rapid streps, blood glucose, rapid trich, more
  • Patient Triage
  • Medication administration, suture/staple removal, dressings and dressing changes
  • Immunizations/therapeutic injections
  • Patient Education and encouraging self-management
  • Clinical assessment in emergencies
  • Rooming patients (pediatrics only, adult med as needed) – height, weight, BP, BMI, head circumference measurements
  • EKG’s (pediatrics only, adult med as needed)
  • Coumadin management per protocol (adult med only)

2. Desk: Phone call triage and advice:

  • ER/Hospital phone follow up
  • Chronic disease phone follow up and outreach
  • Obtaining immunizations from other clinics or schools
  • Prescription refills and responding to pharmacy phone calls
  • VNA referrals.

3. Admin:

  • Ordering supplies
  • Checking code cart/bag
  • Running ‘not-up to date’ registries routinely and providing outreach (primary care preventive services, chronic care needs). These registries may be clinic specific (e.g., immunizations, flu vaccine recall list, newborn database, pap database, family planning logs, other chronic diseases)

QUALITY - The extent to which the employee performs assignments accurately and completely, and consistently meets the job requirements with minimal supervisory oversight and follow-up:

  1. Adheres to the center/departmental policies, procedures, and regulations.
  2. Can be depended upon to meet established standards and achieve desired results.
  3. Pays attention to details in performing job.

EFFICIENCY - The extent, to which the employee’s performance demonstrates an effective organization of assignments, meets expectations of volume and takes initiative to improve the systems that are responsible for the delivery of care and services.

  1. Shows ability to prioritize responsibilities in order to achieve desired results in required time frame.
  2. Takes responsibility for delivery of services as part of team effort in unit/department.
  3. Takes initiative in making suggestions for improvement of services.
  4. Meets deadlines without causing undue disruption.
  5. Utilizes human resources, time, and supplies efficiently.

HUMAN RELATIONS/COMMUNICATIONS - The extent, to which the employee contributes to a positive work environment, communicates effectively and courteously and conducts him/herself in a manner consistent with the mission and values of the Dimock Center:

  1. Adheres to The Dimock Center’s policies regarding attendance and tardiness
  2. Follows policies related to work and breaks
  3. Assists in meeting scheduling needs in helping to cover for absences, emergencies, times of heavy workload, holidays, and vacations.
