HIV Clinical Social Worker/Mental Health Therapist

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.

The Community Care Department at the Health Center provides specialized care for those living with HIV/AIDs.

JOB DESCRIPTION Summary/Objective:

The HIV Clinical Social Worker is responsible for coordinating and facilitating therapeutic groups and individual counseling for HIV positive patients. This position works closely with the Clinical Director of HIV services, HIV Program Manager & Adult Medicine Behavioral Health Team to ensure soundness of program/department policies and procedures.

Essential Functions:

1. Provide individual & group therapy weekly/monthly for HIV positive clients receiving services at Dimock. Patient panel minimum 60 to 80 patients

2. Create a curriculum and facilitate patient support groups with ongoing topics i.e. medication adherences, health & wellness, living positively with HIV, disclosure, relationship

3. Follow & monitor clinical trials in the Boston EMA & provide education about participating in clinical research

4. Consult with providers, case management & psychosocial support staff on client issues.

5. Provide assessments, service plans, referrals, and maintain client records while adhering to Dimock policies and client confidentiality.

6. Provide support to clients with disclosure to family members as well as to HIV infected and affected children.

7. Demonstrate knowledge of understanding multi-dimensional losses that an individual endures when HIV/AIDS is a part of their world as well as tangible losses that complicate an individual’s grief.

8. Provides crisis intervention to clients receiving HIV positive test results.

9. Produce timely billing, assist in obtaining Prior Authorizations and reports to program manager.

10. Produce timely and comprehensive narratives for the completion of periodic and annual reporting as required by the HIV/AIDS Bureau (HAB)/HRSA.

11. Conduct MCM/PSA patient chart audits quarterly

12. Participate in conferences, special events, family retreats, gatherings, off hour and weekend events as needed.

13. Participate in all CQI/QA efforts.

14. Perform other duties as required by supervisors.

Work Environment:

This job operates in an office environment.

Physical Demands:

Duties may require standing and walking, or sitting for long periods of time.

Position Type/Expected Hours of Work:

Expected hours are Monday through Friday 9:00 am to 5:00 pm with some off-hour duties depending on the outreach activities/community events.

Required Education and Experience:

1. Graduate degree in Social Work or Counseling

2. License (LICSW, LCSW, LMHC) required; experience in substance abuse and HIV in communities of color; experience working with women and families with HIV; 1 to 2 years’ experience counseling children and in recruiting /conducting children’s groups preferred.

3. Familiarity with community-based agencies serving people of color.

4. Good community and interpersonal skills.

5. Bilingual/bicultural a plus.

6. Flexibility and ability to work as part of a multidisciplinary team.
