Nurse Practitioner - Adult Medicine

Our Health Center offers a range of primary and specialty health care for adults and children. We provide specialized clinics, Eye Care, Dental, Pediatrics, and OB/GYN.

JOB DESCRIPTION

Summary/Objective:

The Nurse Practitioner will operate in a team based, patient centered adult medicine integrated department. The Practitioner will be responsible for direct patient care activities.

Essential Functions

1. ­­Provides direct patient care delivery for primary care, urgent care/acute care needs.

2. Reviews and signs off all laboratory and imaging results from patients. Works with team on systems for follow up of any abnormal result, communication of normal results, and management plan for “no show” labs/imaging.

3. Reviews and signs off on all transition of care documentation (ED, hospital, referrals) from patients. Works with team on systems for follow up of any outstanding needs, transitional needs, and management plan for “no show” referrals.

4. Provides communication back to primary care team regarding urgent care issues.

5. For purposes of seamless communication and data collection, clearly documents all clinical interactions (in person and by phone) in the electronic health record in accordance with health center and clinic documentation workflows.

6. Ensures that the patients’ needs are at the center of all treatment plans and broader clinical operations plans, and actively involves patients in development of such plans.

7. Ensures maintenance of own knowledge base. Ensures that own interpersonal skills with patients and other clinical staff is always professional and courteous.

8. Works with team members to enhance their skills and plan for continuing training in clinical and customer service areas. Helps to highlight to team the big picture of how the department runs smoothly.

9. Ensures that staff interpersonal skills with patients and providers are always professional and courteous, and that team operations are systematic and efficient.

10. Maintains communications with clinical director regarding performance of care team. Displays clear knowledge of process to communicate to appropriate administrators all performance issues for team members.

11. Accountable for individual and team quality measures. Participates in team or department discussions to optimize data documentation and to take data/measurement to design improvements to processes.

12. Assists in developing interventions in current systems/processes that lead to improved outcomes for patient, populations, and clinic operations.

13. Other duties as assigned.

Work Environment
This position will operate in a clinical setting.

Physical Demands

This position requires the ability to lift and carry up to 20 pounds. Frequent standing, walking, and carrying required.

Position Type/Expected Hours of Work
This position typically operates on a 37.5-hour workweek. One Saturday morning and on call every 5-8 weeks. No nights or holidays.

Required Education and Experience

1. Advance practice nursing degree required.

2. Experience as a licensed independent provider working with diverse populations preferred.

3. Must possess active Massachusetts license for advance practice nursing, as well as DEA and Mass Controlled Substance certifications.

4. Basic Life Support required within 90 days of start.
