Case Manager (Clinical Stabilization Services)

Our Behavioral Health Services programs provide residential and outpatient treatment for those struggling with mental illness, developmental disabilities, or substance use disorders. For many individuals, our programs literally save lives.

Case Manager- Clinical Stabilization Services

The Case Manager is an essential part of a multidisciplinary team of medical professionals, clinicians, counselors and Recovery Specialists who provide support, guidance and community referrals to individuals with substance use and co-occurring disorders, including implementing and providing aftercare case management planning services. The Case Manager is responsible for routinely meeting with, and following up with patients in Clinical Stabilization Services; leading groups and collaborating with all members of patients’ care team.

Essential Functions:

  • Under the guidance of the Clinical Director, engage patients in information gathering and decision making to inform the development of a treatment plan, the setting of treatment goals, determination of service needs, and aftercare planning.
  • Coordinate services with external treatment providers, outpatient counselors / therapists, medication-assisted treatment providers, and other partners such as: primary care providers, DCF, legal, and assist patients to coordinate and attend appointments as needed during their stay.
  • Develop aftercare plans that are in line with patient needs, interests, and eligibility. Make referrals, facilitate interviews, secure placement, and coordinate transition to aftercare services.
  • Provide crisis intervention, escalating to the appropriate party when assistance is needed (Clinical Director, nurse management, counselor, etc).
  • Work collaboratively with the multidisciplinary team to identify changes in patient status, progress made, and development of new needs, and needed changes to the treatment plan.
  • Document all services provided and patient response to those services in the patient chart in accordance with agency and DPH/BSAS guidelines for format, and timeliness; file patient charts with all pertinent documentation at the end of each business day.
  • Conduct and document post-discharge follow-up for clients within 30 days of discharge.
  • Perform all job functions in cooperation with the Director, members of the case management team, Recovery Specialists and other service providers involved in patients’ treatment.
  • Develop supervision plan with Clinical Director and meet on a regular basis to discuss clinical, organizational, and agency issues and review performance.
  • Adhere to all organizational, federal and state regulations/guidelines.
  • Other duties as assigned.
  • Develop and maintain respectful, effective and professional working relationships with other Dimock staff.
  • Consistently demonstrate respect for patients and commitment to their overall growth.
  • Demonstrate working knowledge of Dimock’s continuum of care and facilitates appropriate referrals/contacts within the larger system.
  • Demonstrate knowledge of and adherence to Dimock’s Policies and Procedures.
  • Demonstrate knowledge of and adherence to initiatives in Corporate Compliance and HIPAA.

Required Education and Experience

  • H.S. Diploma or equivalent
  • Minimum one (1) year experience working in a substance use disorder treatment setting

Preferred Experience

  • At least two (2) years experience working in a substance use disorder treatment program and/or with dual-diagnosis individuals

Knowledge / Skills Required

  • Excellent verbal and written communication skills
  • Ability to communicate in a professional manner to patients, leadership, members of the treatment team and other professionals involved in patient care.
  • Ability to develop collaborative relationships with all members of the treatment team and other professionals involved in patient care.
  • Intermediate computer skills (Work, Excel, PowerPoint)
  • Ability to be flexible and juggle multiple priorities
