HIV Psychosocial Support Advocate

Location: Roxbury, MA

JOB DESCRIPTION

Summary/Objective:

The Psychosocial Support Advocate provides Psychosocial Support Services to individuals living with HIV/AIDS. In addition, the psychosocial support and/or peer advocate (person living with HIV) develops on-going relationship with each client; this includes accompanying clients to appointments as necessary (i.e. medical, housing, court, financial), reminder calls and prepare client for interactions with service providers and support clients in those interactions.


Essential Functions:

  1. Complete intake and assessment of clients per case management/PSA guidelines.
  2. Develop and implement treatment plans.
  3. Develop a network of social service providers.
  4. Update psychosocial support client charts on a continuous basis & chart audit review on entire caseload quarterly.
  5. Monitor/update Excel spread sheet daily.
  6. Monitor ECW for upcoming client appointment daily.
  7. Perform patient reminder calls.
  8. Manage all related documentation relevant to client services.
  9. Facilitate support groups
  10. Monitor patient CD4 and VL.
  11. Consumer Advisory Board membership.
  12. Communicate with Medical Case Management staff of shared client caseloads on a continues basis.
  13. Assist MCM staff with client case loads

Work Environment:
This job operates in an office environment, with some out of office client home visits as needed.

Physical Demands:

Duties may require standing and walking, or sitting for long periods of time.

Position Type/Expected Hours of Work:

Expected hours are Monday through Friday 9:00 am to 5:00 pm

Required Education and Experience:

  1. Required: High School Diploma/GED
  2. Support service or case management experience preferred
  3. Working knowledge of serving patients with HIV/AIDS and addiction.
  4. Experience working with culturally diverse communities.
  5. Familiarity with community-based agencies serving people of color.
  6. Bilingual/bicultural a plus.

Salary Range: $26.42 per hour
